  • Letter of Intent
  • PSA-Authenticated Birth Certificate
  • True copy of Grades or Transcript of Records from previous formal schooling (Honorable Dismissal/Transfer Credential) – original and 2 photocopies
  • Resumé with details of job description
  • Notarized Service Record/Certificate of Employment
  • Certificates of training, awards, participation in conferences and workshops as evidence of industry-related experience
  • Four 1”x1” ID Pictures

Procedure

  1. Step 1 Application: The Applicant downloads the ETEEAP Application Form from the PWU ETEEAP website; submit the Accomplished Form with supporting evidence and submits to the ETEEAP coordinator.
  2. Step 2 Screening: The ETEEAP Coordinator reviews the application, interviews the Applicant and informs him/her of the result of the review/interview.
  3. Step 3 Provision for Performance Portfolio: If Accepted, ETEEAP office will require the Applicant to submit his/her performance portfolio with the assistance of the ETEEAP Coordinator. This include descriptions of learning and professional experiences. The Employer concerned should certify these documents.
  4. Step 4 Registration: Formal registration is required to be officially enrolled. The Applicant goes to the Registrar’s office; submits the Registration Form and all the requirements, pays the tuition fees at the Accounting Office.
  5. Step 5 Enhancement: The Applicant takes the courses at PWU through the guidance of his/her ETEEAP Faculty.
  6. Step 6 Final Evaluation: Upon completing all the program academic and professional requirements, the Applicant is awarded the appropriate equivalency credit for the academic degree.
